Request form drinks food and hospitality license

To apply for an Operational License for one of the above mentioned this application form should be filled out completely and include all required documentation. As attachment the following files will be added to this request form: requirements from the Health Inspector and the Fire Department and draft floorplan. Incomplete forms will not be accepted nor processed.
